Antiprotozoal activities of phospholipid analogues

The antiprotozoal activity of phospholipid analogues, originally developed as anti-cancer drugs, has been determined in the past decade. The most susceptible parasites are Leishmania spp. and Trypanosoma cruzi with activity also shown against Trypanosoma brucei spp., Entamoeba histolytica and Acanthamoeba spp. Miltefosine, an alkylphosphocholine, was registered for the oral treatment of visceral leishmaniasis (VL) in India in March 2002. This review will focus on the biological activities of phospholipid analogues. Biochemical and molecular targets and mechanism(s) of action have been studied extensively in tumor cells but have not been determined in protozoa. © 2002 Elsevier Science B.V. All rights reserved.

COMPARISON OF GENERIC TO BRANDED PENTAVALENT ANTIMONY FOR

The cost of generic pentavalent antimony (generic stibogluconate) is approximately one-sixth that of branded pentavalent antimony (stibogluconate in the form of Pentostam or meglumine antimoniate in the form of Glucantime). We compared generic stibogluconate to Pentostam and Glucantime for the treatment of cutaneous leishmaniasis patients in Bolivia and Colombia. For all 114 patients, the per-protocol cure rates were 83-91% and the intent-to-treat cure rates were 75-83%. The highest values were in the generic stibogluconate group. The incidence of pancreatic enzyme abnormalities was 48-88% and the incidence of liver enzyme abnormalities was 48-87%. The lowest incidences were in the generic stibogluconate group. The efficacy and tolerance of inexpensive generic stibogluconate appears comparable to branded formulations for the treatment of cutaneous leishmaniasis in these endemic regions.

Efficacy and Tolerability of Miltefosine for Childhood

Miltefosine has previously been shown to cure 97% of cases of visceral leishmaniasis (VL) in Indian adults. Because approximately one-half of cases of VL occur in children, we evaluated use of the adult dosage of miltefosin (2.5 mg/kg per day for 28 days) in 80 Indian children (age, 2-11 years) with parasitologically confirmed infection in an open-label clinical trial. Clinical and parasitological parameters were reassessed at the end of treatment and 6 months later. One patient died of intercurrent pneumonia on day 6. The other 79 patients demonstrated no parasites after treatment, had marked clinical improvement, and were deemed initially cured. Three patients had relapse, and 1 patient was lost to follow-up. The final cure rate was 94% for all enrolled patients and 95% for evaluable patients.

Miltefosina oral para el tratamiento de la leishmaniasis

La pérdida de la eficacia, las dificultades para su administración y el incremento en la frecuencia y la severidad de los efectos secundarios de las sales de antimonio pentavalente han forzado la búsqueda de un medicamento que pueda remplazarlas en el tratamiento de la leishmaniasis.

Estado actual y futuro de la terapia anti-leishmaniásica en Colombia

Los antimoniales pentavalentes son la primera elección para el tratamiento de la leishmaniasis en Colombia pero la dosis ha tenido que incrementarse en mas de 600% para mantener su eficacia que en distintos reportes varía entre 93 y 25%, lo cual puede reflejar diferencias en la sensibilidad de los parásitos pero más probablemente incumplimiento de los esquemas recomendados. Los resultados de su empleo, así como la situación actual de pentamidina, anfotericina B y miltefosina son revisados y se hace una proyección sobre el desarrollo de la terapia en los próximos años.
